The Department of Transportation requires all Commercial Drivers license (CDL)  holders to receive physical examinations or medical examinations (at least every 2 years) to ensure drivers can safely operate a commercial motor vehicle.  Drivers who meet these requirements will need a medical certificate:

  • Class A:  Any combination of vehicle with a Gross Vehicle Weight Rating (GVWR) of 26,001 lbs or greater,  with towed weight in excess of 10,000 lbs (rated weight)
  • Class B:  Any combination of vehicle with a GVWR of 26,001 lbs or greater,  with towed weight not in excess of 10,000 lbs (rated weight)
  • Class C: Any single vehicle, or combination that does not meet Class A or B, but transport 15 or more passengers plus the driver, or you transport hazardous materials in quantities requiring placards, in interstate commerce.


The Role of the Medical Examiner is to examine the driver and make a determination as to whether they can operate a commercial motor vehicle safely utilizing the guidelines put forth by the Federal Motor Carrier Safety Administration (FMCSA).  The requirements to pass a physical for the DOT are set at a high standard to insure those who drive heavily loaded vehicles will not cause harm to the public.

At Two Rivers Occupational Works we offer DOT or CDL Physicals / Medical exams  at our conveniently located office or onsite for 10+ exams; reducing lost work time for companies.  Your visit to our office for a DOT physical should be quick and easy and typically takes less than 30 minutes.  While the concern of passing the exam may be stressful for some individuals;  there is no need to be stressed over it.   The goal of the exam is not to fail drivers, and in the case an individual doesn't meet the requirements we will work with the individual and treating physicians to the best of our abilities to get them back on the road again.  Walk-ins are available if we have an opening, or to eliminate wait time scheduling is recommended. 

What to expect on your visit

  1. Fill out a 1 -page brief health history
  2. Complete your DOT Physical Exam
  •      Review of medical history  
  •      Physical Exam
  •      Urinalysis (blood, glucose, protein, SG)

DOT Physical Check List 

  • Drivers License
  • List of Medications including regularly used OTC/supplement (This list should include Dosage, treatment being used for,  name of prescribing doctor with contact information)
  • Glasses or contacts if needed (be sure to bring a second pair of glasses just to show doctor you have back-up pair (even if you wear contacts)) 
  • Cutting back/avoiding caffeine and nicotine prior to appointment can help improve blood pressure
  • Be prepared for urinalysis (drink water) 
  • Depending on Health History Clearance or Test results from treating physician may be required (if there is a condition in question; feel free to call the office we would be glad to let you know what might be needed prior to the examination)
